Introduction

Vertical mattress stitch is a suturing technique that uses a deep and superficial stitch (far–far and near–near) to provide strong wound closure and proper eversion of skin edges.

  • Closure of deep wounds

  • High-tension wound areas

  • Promotes wound edge eversion

  • Prevents wound dehiscence

  • Common in back, neck and joints


Principle

The stitch involves two parts:

  • Deep bite (far–far) for strength

  • Superficial bite (near–near) for edge approximation


Procedure Steps

  1. Insert needle 5–10 mm from wound edge (deep bite)

  2. Pass through to opposite side

  3. Reinsert needle 1–2 mm from edge (superficial bite)

  4. Pass through opposite side

  5. Tie knot gently

  6. Ensure edges are everted


Advantages

  • Strong wound closure

  • Good for high-tension wounds
