Intramuscular (IM) Injection – Procedure, Sites, Steps & Nursing Care

Introduction

Intramuscular (IM) injection is a common nursing procedure used to administer medications deep into the muscle. It allows faster absorption than subcutaneous injections. For GNM and BSc Nursing students, proper knowledge of IM injection technique is essential for exams and clinical practice.


What is IM Injection? 

Intramuscular injection is the administration of medication directly into muscle tissue using a syringe and needle for rapid absorption into the bloodstream.


Common Sites for IM Injection

  1. Deltoid muscle – upper arm (small volume)

  2. Vastus lateralis – thigh (preferred in infants)

  3. Ventrogluteal site – safest site in adults

  4. Dorsogluteal site – buttock (less preferred due to nerve risk)


Equipment Required

  • Syringe and needle

  • Medication vial/ampoule

  • Gloves

  • Antiseptic swab (spirit/alcohol)

  • Cotton/gauze

  • Sharps disposal container


Procedure Steps

  1. Verify doctor’s order

  2. Perform hand hygiene

  3. Prepare medication using aseptic technique

  4. Identify correct patient (5 rights)

  5. Select appropriate site

  6. Position patient comfortably

  7. Clean site with antiseptic

  8. Stretch skin or use Z-track technique

  9. Insert needle at 90° angle

  10. Aspirate (if required by protocol)

  11. Inject medication slowly

  12. Withdraw needle quickly

  13. Apply gentle pressure

  14. Dispose needle safely

  15. Document procedure


Z-Track Technique

  • Pull skin sideways before injection

  • Inject medication

  • Release skin after withdrawing needle

  • Prevents leakage and irritation

  • Pain at site

  • Infection

  • Abscess formation

  • Nerve injury

  • Hematoma
